Paracetamol is metabolised primarily in the liver, principally by three metabolic pathways: Glucuronidation, Sulfation (sulfate conjugation) and N-hydroxylation, rearrangement, followed by GSH conjugation. All three pathways yield final products that are inactive, non-toxic, and eventually excreted by the kidneys. In the third pathway, however, the intermediate product NAPQI is toxic.

It is VERY possible to swallow something that BURNS calories in order to detoxify and excrete, that is paracetamol has negative calories.

That is the simple picture. It gets much more complex if we consider what our gut micro-organisms might do to it. (They could digest it into something that does have calories). If you swallow enough of it for this to be significant, you most likely have overdosed.
