It is the case where you need to replace your memory card. It could be a old one. Its circuits have gone off. Your memory card is same like a compact drive, both are storage devices. This problem occurs with both the devices. The memory allocation units of you memory card could be damaged by a virus, or it could be even a physical damage. You might have rubbed it some where or scrattched it somewhere without your knowledge.
If youv'e got yourself a new sdhc or micro sdhc they wont work on older readers. I had this problem and have just purchased a new reader (quite cheap). If you have a different problem then ignore this answer.
If your in windows7 just open disk management and delete the drive then create a new simple drive in its place.
Get it repaired or buy a new one.
No product in the market is perfect.
