Of course, there are different sizes and shapes. Look in the plumbing section of Home Depot or Lowe's and you'll see.
One of the brands is American Standard so their sizes are just a little different. Round and elongated bowl shape. Height comes in an easier higher version now too. Big people still have similar openings positions so they just have to make do I imagine.

The common size for toilet is 12" rough-in. But there are toilets with a 10" or 14" rough-in as well. Typically, the floor mounted toilets can handle the weight up to about 1,000 lbs., but the ones that are hung on the wall can handle only about 500 lbs.
No. There are bowls that are "round" and bowls that are "elongated". And that's just for the bowl. The tanks can vary in shape and volume as well. I know my sheet.
